Nursing care of a patient before operation with mechanical circulation-assisted bridging heart transplantation

To summarize the nursing care of a patient with acute myocardial infarction complicated with cardiogenic shock before operation. The main nursing points are as follows:in the acute stage,the integrated rescue was implemented with rapid on-machine coordination of external cardiopulmonary resuscitation,multidisciplinary collaboration to ensure safe patient transport;during the bridging period,the combined operation care of extracorporeal membrane oxygenation,intra-aortic balloon counter pulsation and continuous kidney replacement therapy was carried out with goal-oriented anticoagulation care,prevention of catheter-related infection with the assistance of mechanical circulation,and neurological function monitoring. The patient successfully passed the waiting period for heart transplantation and underwent heart transplantation 21 days after admission. With the follow-up for 1 year,the patient recovered well.

Construction of nursing quality evaluation indicators in perioperative period of heart transplantation

Objective To construct quality evaluation indicators for perioperative nursing in heart transplantation,and to provide standard and professional quantitative bases for monitoring and management of perioperative nursing quality. Methods This study was conducted based on the frame work of the three-dimensional “structure-process-outcome” quality model,using literature review,Delphi method and analytic hierarchy to determine the content of the indicators,and the weight of each index. Results A total of 22 experts from 14 qualified heart transplantation hospitals were included,and a total of 2 rounds of consultations were conducted. The effective recovery rates of 2 rounds of expert consultation questionnaires were 100%. The authority coefficients were 0.817. The variation coefficients of each item ranged from 0.025~0.169 and 0.039~0.157. The Kendall harmony coefficients were 0.126 and 0.225(P<0.001). The final evaluation indicators for perioperative nursing quality in heart transplantation included 3 first-level indicators,12 second-lever indicators and 59 third-level indicators. Conclusion The evaluation indicators of perioperative nursing quality in heart transplantation was scientific,comprehensive and specialized,which can provide references for the evaluation of perioperative nursing quality in heart transplantation.

To summarize the nursing experience of a post-operational patient with valvular heart disease and prolonged weaning due to tracheomalacia. Key points of nursing include real-time monitoring of bleeding and coagulation,dynamic implementation of anticoagulation programs;strengthening airway management,early warning and treatment of airway collapse;multi-dimensional joint diagnosis and treatment to speed up the rehabilitation process;real-time assessment,sequential management of sedation,analgesia and nutrition programs. After 27 days of careful treatment and nursing,the patient was weaned from a ventilator successfully and transferred to another department,and was discharged after 5 days.

Objective To investigate the effect of specialty nurse-led discharge preparation in the perioperative period of lung transplantation patients. Methods Based on the transition theory,literature analysis and expert meetings,the hospital discharge preparation service plan for lung transplant patients was established. We selected 33 lung transplant patients in a tertiary hospital in Zhejiang Province for this study based on convenience sampling,and they were divided into an experimental group and a control group by random number method. The experimental group received the discharge preparation service program led by specialty nurses,and the control group received routine perioperative management of lung transplantation. Data from admission to 3 months after discharge were collected. Between the 2 groups,they were compared on readiness for hospital discharge,reintubation,readmission,and part 2 of Partners at Care Transitions Measure(PACT-M2). Results A statistically significant difference was found in the scores for readiness for hospital discharge,reintubation rate,and Chinese version PACT-M2(P<0.05). There was no significant difference in the number of readmissions 1 month and 3 months after discharge between the 2 groups(P>0.05). Conclusion Specialized nurse-led discharge preparation services for lung transplant patients can improve their readiness for discharge and enhance the quality of self-care at home.

To summarize the nursing experience of 2 cases of lung transplantation in patients with versus host disease after allogeneic hematopoietic stem cell transplantation. Nursing points include observation and prevention of rejection reaction;strictly implementing prevention and control measures to reduce the risk of infection;through various rehabilitation training,promoting the recovery of respiratory function;conducting nutritional screening and developing nutritional support programmes;timely assessment of psychological state,individual psychological counseling. With aggressive treatment and care,2 patients recovered well and were transferred out of the intensive care unit.

To summarize the first aid and nursing experience of an ICU patient with stress cardiomyopathy after kidney transplantation. Nursing points:focus on changes in the condition and implement efficient first aid measures; rapid and accurate assessment,safe and stable transport;strict hemodynamic measurement,goal-oriented fluid management;precise drug administration,observation of drug effects and adverse reactions; emphasis on psychological counseling and rehabilitation training and doing a good job of continuous nursing are keys to the smooth transfer of patients. After careful treatment and care,the patient was transferred to the kidney transplant ward for continuous specialist treatment on the 16th day after the operation. On the 38th day after the operation,he recovered and was discharged from the hospital.

To summarize the nursing care of 2 patients with prolonged weaning due to early infection with herpes zoster virus after heart transplantation. Nursing points include the establishment of a transplantation management team to refine the management throughout the whole process;the strict prevention and control of the nosocomial infection,systemic antivirus therapy,skin management,analgesia and sleep management for the 2 stages of herpes development;step analgesia and multidisciplinary cooperation for rehabilitation training during healing period. After active treatment and nursing care,the 2 patients were weaned successfully from the ventilator and transferred to a general department. At present,they have recovered and are discharged from the hospital.
